Should I apply for health care coverage renewal?
If you are eligible to renew your health care coverage, you鈥檒l receive a renewal letter in the mail from your state Medicaid agency. Complete this renewal form and return it right away to help avoid a gap in Medicaid or CHIP coverage. Contact your state Medicaid office with any questions or concerns regarding your renewal.
What if I lose health care coverage?
A loss of health care coverage is concerning. You or your child still may be eligible for coverage through your employer or through the Health Insurance Marketplace.聽Call for details.
